Summary

A subgingival orthodontic separator caused localized periodontitis and severe periodontal breakdown in a patient. This case highlights the potential risks of foreign objects in periodontal health.

Background:

Periodontitis is a common inflammatory condition affecting the gums and supporting bone.
Orthodontic separators are used to create space between teeth prior to orthodontic treatment.
Subgingival placement of foreign objects can pose risks to periodontal health.

Observation:

A case study involving a patient who developed localized periodontitis was presented.
The periodontitis was specifically associated with a subgingival orthodontic separator placed below the gum line.

Findings:

The presence of the subgingival orthodontic separator was directly linked to severe periodontal tissue destruction.
The affected area was around an upper right first premolar tooth.

Implications:

This case underscores the potential for iatrogenic periodontal damage from orthodontic appliances.
Clinicians must exercise caution during separator placement to avoid subgingival impaction and subsequent periodontal complications.
