Summary

Quantitative sensory testing (QST) can detect genital sensory deficits in multiple sclerosis (MS) patients, correlating with female sexual dysfunction (FSD). Clitoral vibration testing shows promise for identifying FSD in MS.

Background:

  • Female sexual dysfunction (FSD) affects 45-74% of women with multiple sclerosis (MS).
  • Quantitative sensory testing (QST) assesses neural function in the female genitalia.
  • The neural basis of FSD in MS patients requires further investigation.

Purpose of the Study:

  • To assess genital neural function in female MS patients using QST.
  • To investigate the correlation between sensory deficits and FSD in MS.
  • To explore the role of the cerebellum in sexual function (SF) in MS.

Main Methods:

  • 41 female MS patients (aged 21-56) with a median disease duration of 10 years were studied.
  • Neurological examination, SF questionnaires, and focused interviews were conducted.
  • QST assessed clitoral and vaginal temperature and vibratory sensation.

Main Results:

  • 61% of patients reported FSD, with decreased libido and orgasmic disturbances being most common.
  • Significant correlations were found between sensory deficits and FSD parameters.
  • Clitoral vibratory sensation showed a strong correlation with orgasmic dysfunction (r = 0.423, P = 0.006).
  • Cerebellar deficits were significantly associated with orgasmic dysfunction (P = 0.0012).

Conclusions:

  • QST, particularly clitoral vibration testing, may be a valuable tool for detecting FSD in MS patients.
  • The findings support a significant role for the cerebellum in female sexual function.
