Summary

This study shows a successful oxaliplatin desensitization protocol without premedication for a patient with hypersensitivity reactions. This approach may help patients avoid cumulative exposure to pretreatment medications during chemotherapy.

Background:

  • Oxaliplatin, a platinum-based chemotherapy agent, is used for colorectal carcinoma.
  • Hypersensitivity reactions to oxaliplatin occur in 12-16% of patients.
  • Previous desensitization protocols often involve premedication with steroids or antihistamines.

Observation:

  • A 43-year-old male with metastatic rectal adenocarcinoma developed urticaria after oxaliplatin infusion.
  • Skin prick and intracutaneous testing for oxaliplatin hypersensitivity were negative.
  • A novel desensitization protocol without premedication was initiated.

Findings:

  • The patient received oxaliplatin desensitization starting at a 1:10,000 dose, gradually increasing to the therapeutic dose.
  • The protocol was successfully repeated for 11 cycles without hypersensitivity reactions.
