Background:

Pleural adenocarcinoma is a rare malignancy.
Identifying the primary tumor origin can be challenging.

Observation:

A 58-year-old male presented with a large, localized pleural tumor involving the chest wall and ribs.
Initial biopsy confirmed adenocarcinoma, but extensive surveys found no other primary neoplasm.
Surgical resection of the chest wall and pleuropneumonectomy were performed due to pleural dissemination.

Findings:

Pathological examination revealed no primary lung tumor.
Immunohistochemistry suggested a subpleural lung origin with chest wall invasion.
The tumor may represent a variant of pseudomesotheliomatous adenocarcinoma.
