Background:

Medication errors pose a significant risk in healthcare settings.
Computerized prescriber order entry (CPOE) systems aim to reduce errors but can introduce new challenges during implementation.
The role of pharmacy personnel in CPOE adoption is crucial for success.

Purpose of the Study:

To compare medication error rates and types before and after CPOE implementation.
To describe the involvement of a pharmacy resident in the CPOE implementation process.
To analyze the impact of CPOE on medication error reporting.

Main Methods:

CPOE was implemented in a neurosurgical intensive care unit (ICU).
Medication errors were tracked and compared for two years pre- and post-implementation.
Error types, severity, and reporting personnel were analyzed.

Main Results:

A fivefold increase in ordering errors was observed post-CPOE implementation.
Most errors did not cause patient harm.
Pharmacy residents reported a significantly higher number of errors compared to previous pharmacist reporting.

Conclusions:

An increase in reported medication errors is expected during the initial CPOE transition.
Pharmacy departments and residents significantly influence CPOE implementation safety and efficiency.
Active pharmacy involvement is vital for successful CPOE adoption.
