Summary

Brainstem abscesses are rare but treatable with advanced imaging. A patient with a large pontomesencephalic abscess showed significant improvement with antibiotics and MRI monitoring.

Background:

  • Brainstem abscesses are historically rare and often fatal.
  • Advances in computed tomography (CT) and magnetic resonance imaging (MRI) have improved diagnosis and management.
  • The brainstem presents unique challenges for treating abscesses due to its critical structures.

Observation:

  • A 51-year-old patient presented with dizziness, headache, cranial nerve palsies, ophthalmoplegia, cerebellar syndrome, and contralateral hemiplegia.
  • A widespread pontomesencephalic abscess was diagnosed.
  • Microbiologic investigations were negative.

Findings:

  • The patient was treated with broad-spectrum antibiotics and periodic MRI.
  • The abscess size decreased from 4 cm to 0.5 cm.

    • Empirical medical therapy, including antibiotics, can be effective for brainstem abscesses, even large ones.
    • Stereotactic drainage is a common treatment, but medical management alone can succeed in select cases.
