Background:

  • Coronaviruses typically cause upper respiratory tract infections.
  • Gastroenteritis is a known manifestation in Human Immunodeficiency Virus (HIV) patients.
  • Limited data exists on respiratory infections caused by coronaviruses in HIV patients, barring Severe Acute Respiratory Syndrome (SARS).

Observation:

  • Two HIV-positive patients experienced exacerbations of chronic obstructive pulmonary disease (COPD) with confirmed coronavirus infection.
  • A third HIV-positive patient presented with an upper respiratory tract infection, though coronavirus was not isolated.
  • All three individuals shared a common exposure event four days before symptom onset.

Findings:

  • This case series represents the first documented instances of respiratory tract infection with coronavirus in patients with HIV.
  • Coronavirus infections in these HIV patients led to prolonged hospitalizations and intensive diagnostic workups.
  • The severity of illness in these cases suggests coronaviruses can cause significant respiratory disease in immunocompromised individuals.

Implications:

  • Coronavirus should be considered in the differential diagnosis of respiratory tract infections in patients with HIV.
  • The findings emphasize the potential for coronaviruses to cause severe respiratory illness in immunocompromised populations.
  • Further research is warranted to understand the spectrum and management of coronavirus infections in HIV patients.
