Summary

Sebaceous carcinoma, an aggressive skin cancer, was unexpectedly diagnosed in a patient with actinic damage. Surgical excision was successful, with no recurrence observed after four years.

Background:

  • Actinic damage is a common precursor to skin malignancies.
  • Squamous cell carcinoma (SCC) and solar keratoses are frequent sequelae of sun exposure.
  • Mr LA presented with persistent actinic lesions refractory to cryotherapy.

Observation:

  • Two actinic lesions on the left cheek did not respond to cryotherapy.
  • Histopathological examination revealed sebaceous carcinoma, an uncommon and aggressive cutaneous malignancy.
  • The 5-year survival rate for sebaceous carcinoma is approximately 60-70%.

Findings:

  • The sebaceous carcinoma was widely excised with a 10 mm margin.
  • A multidisciplinary team decided against adjuvant radiotherapy.
  • The surgical wound healed well within 8 weeks.

Implications:

  • Complete surgical excision can be an effective treatment for sebaceous carcinoma.
  • Close monitoring is crucial due to the aggressive nature of the tumor.
  • This case highlights the importance of thorough histopathological evaluation for persistent skin lesions.
