Summary

This case report details a rare chronic neutrophilic leukemia presentation. The patient initially showed Sweet's syndrome, followed by leukocytosis, highlighting an unusual disease progression.

Background:

  • Chronic neutrophilic leukemia (CNL) is a rare myeloproliferative neoplasm.
  • CNL is defined by splenomegaly, sustained neutrophilia, elevated vitamin B12, and absence of the Philadelphia chromosome.

Observation:

  • A 74-year-old male presented with Sweet's syndrome, a neutrophilic dermatosis.
  • The patient subsequently developed leukocytosis, consistent with a myeloproliferative process.

Findings:

  • The patient exhibited splenomegaly, elevated vitamin B12, serum uric acid, and neutrophil alkaline phosphatase.
  • Cytogenetic studies were normal, and the BCR-ABL gene transcript was undetectable, ruling out chronic myeloid leukemia.
