This study presents a case of pontocerebellar hypoplasia type 1 in a child with severe psychomotor retardation. Mitochondrial respiratory chain studies are suggested to further understand the role of mitochondrial dysfunction in this rare neurodegenerative disorder.

- Genetic testing for spinal muscular atrophy was negative.

- The reported case aligns with pontocerebellar hypoplasia type 1.
- Mitochondrial respiratory chain investigations are valuable for elucidating the pathogenesis of pontocerebellar hypoplasia.
