Summary

Procainamide treatment for cardiac arrhythmias frequently causes antinuclear antibodies and histone antibodies. These serologic abnormalities, linked to procainamide dosage, may indicate an increased risk for drug-related lupus.

  • Main Results:

    • Antinuclear antibodies (ANA) detected in 83% of patients.
    • Significant elevations in antibodies to ssDNA, dsDNA, Z-DNA, and poly A (P < 0.001).
    • Histone antibodies found in 65.4%, with H2A/2B dimer being most frequent. IgG anti-H2A/2B correlated with procainamide dose. One patient developed drug-related lupus.

    Conclusions:

    • Procainamide therapy is associated with a high incidence of serologic abnormalities, including ANA and histone antibodies.
    • The development of these autoantibodies correlates with procainamide dosage.
    • Monitoring for serologic changes may be important in patients on long-term procainamide treatment.
