Thoraco-abdominal approach in tumour nephrectomy.

E Schmiedt, P Carl, K Wanner

    Summary

The thoraco-abdominal surgical approach for renal carcinoma reduces inoperability rates by enabling earlier ligation of the vascular pedicle. While survival rates show no immediate improvement, this method is expected to decrease local recurrence due to enhanced radicality.

    Area of Science:

    • Urology
    • Surgical Oncology
    • Nephrology

    Background:

    • Renal carcinoma poses risks of intraoperative metastasis, venous invasion, and lymph node infiltration.
    • Traditional lumbar surgical approaches may have limitations in managing advanced renal tumors.

    Purpose of the Study:

    • To evaluate the efficacy of a thoraco-abdominal surgical approach for renal carcinoma.
    • To assess the impact of this approach on metastasis, invasion, and inoperability rates.
    • To compare outcomes with the traditional lumbar approach.

    Main Methods:

    • Implementation of a thoraco-abdominal surgical technique for renal carcinoma.
    • En bloc total nephrectomy with adrenal gland and fatty capsule removal.
    • Paracaval, paraaortal, and intravasal lymphadenectomy.
    • Tumor thrombectomy for vena cava involvement.

    Main Results:

    • The inoperability rate was significantly reduced to 2.1% compared to 11.3% with the lumbar approach.
    • No significant improvement in survival rates was observed in the first three postoperative years compared to the lumbar approach.
    • Improved radicality of tumor removal was achieved.

    Conclusions:

    • The thoraco-abdominal approach offers advantages in managing renal carcinoma by minimizing intraoperative risks and reducing inoperability.
    • While short-term survival benefits are not yet evident, the enhanced radicality is anticipated to lower local recurrence rates.
    • This surgical strategy is recommended for preventing metastasis and improving tumor resectability in renal carcinoma cases.
