New electrocardiogram (ECG) variables can automatically detect prior myocardial infarction (MI) regardless of location. These quantitative repolarization measures offer a simpler alternative to conventional ECG methods for identifying old heart attacks.
Area of Science:
- Cardiology
- Medical Imaging
- Biomedical Engineering
Background:
- Prior myocardial infarction (MI) detection is crucial for patient management.
- Conventional electrocardiogram (ECG) methods for identifying old MI can be limited.
- Need for quantitative, automatically applicable ECG variables for MI detection.
Purpose of the Study:
- To identify quantitative, automatically applicable ECG variables for detecting prior MI in different myocardial regions.
- To evaluate the efficacy of these variables compared to the Minnesota code.
Main Methods:
- Observational study involving 144 patients with prior MI and 75 healthy controls.
- Body surface potential mapping (BSPM) recorded at rest and analyzed for ECG parameters.
- MI location determined by cine angiography or echocardiography; 12-lead ECG interpreted using the Minnesota code.
Main Results:
- T-apex amplitude, QRSSTT integral, and STT integral showed high accuracy (AUC >90%) in detecting MI across all locations.
- QRSSTT integral achieved AUC of 93% for anterior MI and 92% for inferoposterior MI.
- These repolarization variables outperformed the Minnesota code and could distinguish between anterior and inferoposterior MI (AUC >85%).
Conclusions:
- Quantitative, automatically applicable single-lead repolarization variables effectively detect prior MI irrespective of location.
- These novel ECG variables may simplify the screening and localization of old infarctions.
- Potential to improve upon conventional ECG methods for MI assessment.
