  • A 10-year-old girl presented with dysphagia and vomiting due to a brainstem epidermoid cyst.
  • Magnetic resonance imaging revealed a cystic mass and hydrocephalus.
  • Initial treatment was for a presumed abscess before definitive diagnosis.

Findings:

  • Despite four interventions, the patient maintained good condition with minimal deficits.
  • Subtotal resection of brainstem epidermoid cysts in children is associated with high recurrence rates.

Implications:

  • Total resection is the optimal treatment strategy for pediatric brainstem epidermoid cysts.
