Musculoskeletal hydatidosis, a rare parasitic infection caused by Echinococcus granulosus, can occur outside typical locations. Advanced serological and molecular tests confirmed a muscular hydatidosis case in a young man, aiding diagnosis.

Background:
- Hydatidosis is a zoonotic disease caused by Echinococcus granulosus, typically presenting as hepatic or pulmonary cysts.

- A 34-year-old man presented with a large cystic mass in his thigh, initially detected via ultrasound and confirmed by MRI.
- Initial serological tests for hydatidosis were negative, but a repeat test showed an increased antibody titer.

- Surgical excision and subsequent molecular analysis of the cyst confirmed Echinococcus granulosus genotype G1.
- Advanced serological tests (Western Blot, REAST) for IgE and IgG hydatidosis antibodies were positive, aiding definitive diagnosis.

- Cystic hydatidosis should be considered in the differential diagnosis of any cystic mass, even in low-prevalence areas.
- Modern molecular and serological techniques are crucial for diagnosing rare presentations of hydatidosis.
