Background:

  • Systemic lupus erythematosus (SLE) is associated with increased cardiovascular mortality, primarily due to accelerated atherosclerosis.
  • Endothelial dysfunction is an early indicator of atherosclerosis and cardiovascular risk.
  • Understanding endothelial function in SLE is crucial for assessing cardiovascular risk.

Purpose of the Study:

  • To investigate endothelial function in female patients with SLE.
  • To compare endothelial function between SLE patients and healthy controls.
  • To assess the relationship between endothelial function and clinical outcomes over five years.

Main Methods:

  • Thirty-six female SLE patients and 22 healthy controls underwent assessment of endothelial-dependent vasodilatation (EDD) via brachial artery response to shear stress.
  • Endothelium-independent dilatation was measured using glyceryl trinitrate.
  • Patients were followed for up to five years to record organ system damage.

Main Results:

  • SLE patients demonstrated significantly impaired EDD compared to controls (5.6% vs. 8.0%, P = 0.001).
  • Endothelium-independent dilatation was similar between groups.
  • Impaired endothelial function was noted in postmenopausal SLE patients and inversely correlated with total cholesterol.

Conclusions:

  • Patients with SLE exhibit impaired endothelial function, suggesting an increased risk for atherosclerosis.
  • Endothelial function was poorer in postmenopausal women with SLE.
  • No correlation was found between endothelial function and the development of organ damage over five years.
