Background:

  • The etiology of lichen planus remains unknown.
  • Previous research indicated the presence of human herpes virus 7 (HHV-7) DNA and proteins in lesional lichen planus skin.
  • Lesional skin in lichen planus patients showed infiltration by plasmacytoid dendritic cells.

Purpose of the Study:

  • To investigate the association between HHV-7 replication and lichen planus.
  • To determine if HHV-7 DNA and protein levels decrease upon clinical remission of lichen planus.
  • To identify the specific cell types involved in HHV-7 expression within lesional skin.

Main Methods:

  • Nested PCR was used for HHV-7 DNA detection.
  • Immunohistochemistry was employed for HHV-7 protein detection.
  • Skin biopsies from lichen planus patients before treatment and after remission were analyzed.

Main Results:

  • HHV-7 DNA was detected in 61% of pre-treatment biopsies versus 8% post-remission (P = 0.01).
  • A significant reduction in HHV-7 protein-positive cells was observed in both the dermis and epidermis after remission.
  • HHV-7 expression was primarily localized to BDCA-2 positive plasmacytoid dendritic cells, not CD-3 positive lymphocytes.

Conclusions:

  • HHV-7 replicates within plasmacytoid dendritic cells in lesional lichen planus skin.
  • HHV-7 presence and replication diminish significantly following lichen planus remission.
  • These findings provide further support for the hypothesis linking HHV-7 to the pathogenesis of lichen planus.
