  • Autofluorescence bronchoscopy was offered to all subjects.
  • Main Results:

    • CT identified numerous small nodules; most were benign and stable over 2 years.
    • Persistent nodule growth on CT increased malignancy probability to 75%.
    • The two-step strategy, including sputum biomarkers, improved lung cancer detection from 3% to 5%.

    Conclusions:

    • While CT detects many nodules, persistent growth is a key indicator of malignancy.
    • Autofluorescence bronchoscopy detected CT-occult cancers, accounting for 25% of diagnoses.
    • Prescreening with sputum biomarkers significantly enhances lung cancer detection rates in high-risk individuals.
