Area of Science:

  • Plastic Surgery
  • Reconstructive Surgery
  • Microsurgery

Background:

  • Congenital breast deformities present a significant reconstructive challenge for plastic surgeons.
  • Common deformities include Poland syndrome, unilateral hypoplasia, tuberous breast anomaly, and amastia.
  • Patients are typically young individuals seeking aesthetic restoration.

Observation:

  • A retrospective review of 12 perforator flap reconstructions for congenital breast deformities between 1994 and 2005.
  • Cases involved Poland syndrome (n=8) and unilateral breast hypoplasia (n=4).
  • Internal mammary vessels were consistently used as recipient vessels.

Findings:

  • No flap loss occurred, and no vein grafts were required.
  • Postoperative complications included seroma, hematoma, and nipple malposition, with 30% requiring revision.

  • Aesthetic outcomes ranged from poor to excellent.
  • Implications:

    • Perforator flaps are a viable autologous reconstruction method for congenital breast deformities.
    • Flap choices like DIEP, SIEA, or GAP depend on tissue availability.
    • This technique offers a safe, reliable option with potential for improved self-esteem and aesthetic results.
