Background:

Frequent hospital readmissions pose a significant challenge in managing chronic illness.
Understanding patient heterogeneity is crucial for developing effective readmission reduction strategies.

Purpose of the Study:

To identify distinct patient subtypes within a chronic illness cohort based on readmission patterns.
To compare factors influencing readmission across identified patient groups.

Main Methods:

A descriptive correlational survey using a structured questionnaire.
Cluster analysis of 74 readmitted patients in Hong Kong.
Analysis of five outcome variables: predisposing characteristics, need factors, health behavior, health status, and enabling resources.

Main Results:

Two patient profiles (clusters) emerged from the analysis.
Cluster A (41.9%) had higher readmission rates (32.3% >1 within 28 days) compared to Cluster B (58.1%, 9.3%).
Cluster A patients were younger, more educated, and received fewer community nurse services than Cluster B patients.

Conclusions:

Community nurse services may reduce readmissions, particularly for patients similar to Cluster B.
Identifying patient profiles enables targeted interventions to reduce unplanned hospital readmissions.
This model can assist hospital managers in designing specific strategies for different patient groups.
