Summary

Blastomycosis is an uncommon infection in solid organ transplant recipients, often presenting as pneumonia and complicated by acute respiratory distress syndrome (ARDS). Lifelong antifungal therapy may not be necessary after successful treatment.

Background:

  • Blastomyces dermatitidis causes severe blastomycosis in immunocompromised individuals.
  • Limited data exists on blastomycosis in solid organ transplant (SOT) recipients.

Purpose of the Study:

  • To describe the epidemiology, clinical features, and outcomes of blastomycosis in SOT recipients.

Main Methods:

  • Retrospective case-series at the University of Wisconsin Hospital and Clinics.
  • Included SOT recipients diagnosed with blastomycosis between 1986 and 2004.

Main Results:

  • 11 cases identified; 64% occurred between 2000-2004.
  • Median onset 26 months post-transplant; pneumonia common, often with ARDS.
  • 36% mortality, rising to 67% with ARDS; extrapulmonary disease involved skin, not CNS.

Conclusions:

  • Blastomycosis is an uncommon but serious SOT complication.
  • Frequent complications include ARDS, dissemination, and co-infections.
  • Lifelong antifungal suppression may not be required post-cure.
