Summary

Vocalizations during seizures do not reliably predict seizure origin. This study found vocalizations occurred equally in seizures starting in dominant or non-dominant temporal lobes.

Purpose of the Study:

  • To investigate the relationship between spontaneous vocalizations during seizures and the anatomical location of seizure onset and spread.
  • To determine if vocalizations can reliably predict the origin or propagation of complex partial seizures.

Main Methods:

  • Retrospective analysis of 22 patients with refractory complex partial seizures.
  • Patients were evaluated using bilateral intracranial electrodes.
  • Vocalization events during seizures were recorded and analyzed in relation to seizure origin.

  • In non-vocalizing patients with localized onsets (6/10), seizures arose from both dominant and non-dominant mesial temporal cortex.

Conclusions:

  • The presence or absence of vocalizations during a seizure is not a reliable indicator of the seizure's starting or spreading regions.
  • Vocalizations do not consistently differentiate between seizures originating in language-dominant versus non-dominant temporal lobes.
