Background:

  • Rigid bronchoscopy can cause significant hemodynamic changes and patient awareness under anesthesia.
  • Anesthetic agents aim to mitigate these responses and prevent awareness.
  • The Bispectral Index (BIS) is used to monitor anesthetic depth and has shown promise in reducing awareness and improving recovery.

Purpose of the Study:

  • To prospectively observe Bispectral Index (BIS) values in patients undergoing rigid bronchoscopy.
  • To evaluate the depth of anesthesia achieved during rigid bronchoscopy using BIS monitoring.

Main Methods:

  • A prospective observational study was conducted on 50 patients.
  • Bispectral Index (BIS) values were continuously monitored during routine anesthesia for rigid bronchoscopy.

Main Results:

  • Patients spent only 0.5% of the bronchoscopy duration with BIS values between 40-60.
  • BIS values were below 40 for 99.6% of the procedure.
  • One instance of possible patient awareness was noted.
  • No significant difference in BIS values was observed between intermittent bolus and target-controlled infusion propofol anesthesia.

Conclusions:

  • The majority of patients undergoing rigid bronchoscopy with general anesthesia had BIS scores below the recommended 40-60 range.
  • BIS < 40 was more common in this population compared to other surgical groups.
  • Current BIS monitoring guidelines may need adjustment for rigid bronchoscopy procedures.
