Summary

Cochlear implantation provides good speech intelligibility for post-lingually deafened adults. Factors like age and duration of deafness have minimal impact, supporting implantation even in older patients or those with long-term hearing loss.

The Cochlea

The cochlea is a coiled structure in the inner ear that contains hair cells—the sensory receptors of the auditory system. Sound waves are transmitted to the cochlea by small bones attached to the eardrum called the ossicles, which vibrate the oval window that leads to the inner ear. This causes fluid in the chambers of the cochlea to move, vibrating the basilar membrane.

  • High average speech intelligibility scores were observed: 85.1% for vowels, 41.1% for CV syllables, and 80.4% for phrases.
  • Weak correlations (0.03 to -0.27) were found between age at operation, duration of deafness, number of external electrodes, and performance.
  • No significant difference in speech intelligibility between patients with cochlear obstruction and open cochlea.

Conclusions:

  • Cochlear implantation yields good speech outcomes in post-lingually deafened individuals.
  • Age, duration of deafness, and external electrode placement have limited predictive value for performance.
  • Cochlear implantation is a viable option for elderly patients, those with long-standing deafness, and cases with cochlear obstruction.
