Graft pneumonectomy and delayed contralateral lung transplantation

• The patient developed severe bronchial anastomosis stenosis and bronchiectasis in the graft.
• Four years post-transplant, a left pneumonectomy was performed due to graft complications.

Findings:

• A delayed contralateral single lung transplantation (right lung) was successfully performed after the left pneumonectomy.
• The patient demonstrated significant recovery, becoming ambulatory and oxygen-independent nine months post-procedure.
• This case represents a potentially viable, albeit complex, salvage strategy for lung transplant recipients with graft failure.

Implications:

• This approach may offer a solution for patients with failing lung transplants who are not candidates for re-transplantation.
• Further research is warranted to establish the safety and efficacy of this technique.
• Contralateral single lung transplantation after graft pneumonectomy expands the options for managing complex lung transplant complications.
