Summary

Repeat hepatectomy for hepatocellular carcinoma (HCC) offers survival benefits, particularly for patients without vascular invasion. However, high re-recurrence rates limit its effectiveness in curing recurrent HCC.

Background:

  • Hepatocellular carcinoma (HCC) recurrence after partial hepatectomy leads to poor long-term prognosis.
  • The effectiveness of repeat hepatectomy for recurrent HCC remains debated.

Purpose of the Study:

  • To retrospectively evaluate the significance and outcomes of repeat hepatectomy in patients with recurrent HCC.

Main Methods:

  • Analysis of survival rates in 84 patients who underwent a second hepatectomy with curative intent between 1990 and 2004.
  • Retrospective review of patient data and outcomes.

Main Results:

  • Overall 5-year survival rate was 50% after the second hepatectomy.
  • Absence of microscopic vascular invasion and recent hepatectomy (1997-2004) were independent prognostic factors for survival.
  • Patients with both factors had an 80% 5-year survival rate, but recurrence-free survival remained low (6%).

Conclusions:

  • Repeat hepatectomy provides survival benefits for recurrent HCC, especially in patients without microscopic vascular invasion.
  • High rates of re-recurrence and short recurrence-free intervals limit the curative potential of repeat hepatectomy.
  • The overall effectiveness of repeat hepatectomy for curing recurrent HCC is limited.
