Background:

  • Primary pulmonary hypertension (PPH) is typically diagnosed in younger adults.
  • The natural course and diagnostic challenges of PPH in elderly individuals (≥65 years) are not well-defined.

Observation:

  • A retrospective review identified 8 elderly patients diagnosed with PPH over an 8-year period.
  • Common presenting symptoms included dyspnea, chest pain, and pedal edema.
  • Initial diagnoses were frequently incorrect, attributing symptoms to more common geriatric conditions.

Findings:

  • Elderly PPH patients exhibited symptoms similar to younger counterparts.
  • Coexisting medical conditions complicated the diagnostic process.
  • Survival was significantly reduced in patients with symptom duration under 6 months.
