Summary

Primary coronary angioplasty (PCA) improves regional heart muscle function after myocardial infarction. However, persistent changes in left ventricular (LV) shape indicate that systolic deformation remains over time.

Background:

  • Acute myocardial infarction (MI) leads to regional left ventricular (LV) contractile dysfunction and remodeling.
  • Primary coronary angioplasty (PCA) is a key intervention for acute MI, but its long-term effects on LV mechanics require detailed evaluation.

Purpose of the Study:

  • To assess the impact of successful primary coronary angioplasty (PCA) on regional left ventricular (LV) contractile dysfunction, deformation, and global remodeling following first myocardial infarction.

Main Methods:

  • Ninety-nine patients with first MI underwent successful PCA of LAD or RCA arteries.
  • LV shape (eccentricity, circularity), wall motion, and regional curvature were analyzed at 1 and 6 months post-procedure.
  • Improvement was defined as normalization of wall motion at 6 months.

Main Results:

  • PCA improved regional contractility in all patients, salvaging injured myocardium and reducing infarct area in some.
  • Despite improved contractility, LV shape remained abnormal in most patients.
  • Global LV remodeling occurred in patients with larger infarcts and higher end-diastolic volumes at presentation.

Conclusions:

  • Primary coronary angioplasty (PCA) effectively reverts regional contractility impairment caused by acute myocardial infarction.
  • Persistent systolic shape deformation of the left ventricle (LV) is observed even after successful revascularization.
