Epilepsy surgery can benefit selected patients with tuberous sclerosis complex (TSC). However, moderate to severe intellectual disability and tonic seizures are linked to recurrence, impacting outcomes for individuals with TSC-related epilepsy.

- 57% of patients achieved complete seizure freedom post-surgery.
- An additional 18% experienced >90% seizure reduction.
- Moderate/severe intellectual disability and tonic seizures were significant predictors of seizure recurrence.
Conclusions:
- The study did not find a link between multiple seizure types, early onset, tubers, or multifocal epileptogenicity and poor surgical outcomes.
- Resective surgery appears beneficial for carefully selected TSC patients with drug-resistant epilepsy.
- Further research may refine patient selection criteria for improved surgical success rates.
