Summary

Gall bladder polyps are usually benign but often malignant in patients with primary sclerosing cholangitis (PSC). Early surgical removal (cholecystectomy) is recommended for any gall bladder polyp in PSC patients, regardless of size.

Background:

  • Gall bladder polyps (PLG) are common, with malignancy risk generally low for those under 1 cm in the general population.
  • Malignancy can arise in gall bladder polyps, necessitating careful evaluation.

Observation:

  • This study investigated four patients with primary sclerosing cholangitis (PSC) who developed gall bladder cancer within a gall bladder polyp.
  • Patients presented with polyps ranging from 7 mm x 8 mm to 25 mm x 14 mm, with normal tumor markers.

Findings:

  • All four patients with PSC and gall bladder polyps were diagnosed with cancer and underwent cholecystectomy.
  • Outcomes varied by tumor stage; early-stage tumors had good prognoses, while advanced tumors required further treatment with varied success.

Implications:

  • Gall bladder polyps in patients with PSC have a higher likelihood of malignancy than in the general population.
  • Prophylactic cholecystectomy should be strongly considered for all gall bladder polyps in PSC patients, irrespective of polyp size.
