    Background:

    • Enteropathy associated T-cell lymphoma (EATCL) is a rare, aggressive non-Hodgkin lymphoma.
    • It is strongly associated with a history of gluten sensitive enteropathy (GSE).

    Observation:

    • This case report details three patients diagnosed with EATCL.
    • Patients presented with varied symptoms including small intestine obstruction, renal failure, and disease progression despite treatment.
    • Diagnostic methods included histopathology and immunohistochemistry, confirming EATCL in all cases.

    Findings:

    • All three patients had positive IgA antiendomysial antibodies.
    • Despite chemotherapy, patients experienced disease progression, metastasis (lung infiltration), and ultimately succumbed to the disease, with an overall survival of 7 months.
    • One patient had no prior history of GSE, suggesting EATCL can occur without a known GSE history.

    Implications:

    • Clinical progression of GSE despite a gluten-free diet warrants consideration for underlying EATCL.
    • Increased awareness and early diagnosis of EATCL in patients with GSE are critical.
    • Further research into EATCL pathogenesis and treatment strategies is needed due to its poor prognosis.
