    Background:

    • The National Hospice and Palliative Care Organization initiated efforts to integrate social work into patient/family outcomes.
    • The Social Work Assessment Tool (SWAT) was developed as part of these initiatives.
    • Previous pilot studies informed the revision and refinement of the SWAT.

    Purpose of the Study:

    • To report on the development and performance of the Social Work Assessment Tool (SWAT).
    • To evaluate the effectiveness of the SWAT in measuring patient and family outcomes in hospice and palliative care settings.
    • To identify areas for improvement in social work outcomes measurement.

    Main Methods:

    • A performance improvement project involving 19 social workers from 14 hospice programs.
    • The SWAT was used with 101 patients and 81 primary caregivers over a median of 44 days.
    • Quantitative analysis of SWAT scores and qualitative interviews with social workers.

    Main Results:

    • Significant improvement in patient SWAT scores was observed between the first and second social work visits (p < .01).
    • Qualitative feedback indicated a need for further training and readiness for quantitative outcomes measurement among social workers.
