Background:

Malignant intracerebral nerve sheath tumors are exceptionally rare in medical literature.
Current therapeutic guidelines for these tumors are absent.
Sarcoma, not otherwise specified (NOS) represents a challenging diagnostic category.

Observation:

A 28-year-old female presented with a left parietal lobe tumor, classified as sarcoma NOS.
The tumor recurred rapidly after initial surgical excision.
The patient received 5,400 cGy radiotherapy in 22 fractions, guided by CT imaging.

Findings:

Two weeks post-radiotherapy, significant tumor regression was observed via MRI.
The patient developed pulmonary metastases, responding partially to chemotherapy.

Interdisciplinary collaboration is crucial for optimizing diagnosis, treatment, and patient outcomes.
