Background:

  • Sarcomatous change in hepatocellular carcinoma (HCC) is uncommon.
  • Anticancer therapy is a suspected, but not the only, cause of sarcomatous transformation.
  • This phenomenon requires further investigation due to its rarity.

Observation:

  • A 79-year-old male presented with a liver tumor.
  • Imaging studies showed conflicting results: cystic on CT/MRI, solid on ultrasound.
  • The patient underwent surgery for suspected atypical HCC.

Findings:

  • Microscopic examination confirmed hepatocellular carcinoma with sarcomatous change.
  • The patient had no history of prior anticancer therapy.
  • This finding challenges the established link between therapy and sarcomatous transformation.

Implications:

  • This case contributes to the understanding of sarcomatous HCC development.
  • It suggests that sarcomatous change can arise independently of previous treatments.
  • Further research is needed to elucidate the mechanisms driving this rare liver cancer subtype.
