Summary

Bilateral vocal cord paralysis in newborns rarely resolves spontaneously. Early surgical intervention, particularly arytenoidopexy combined with botulinum toxin, offers the best outcomes for these infants.

Background:

  • Bilateral vocal cord paralysis (BVCP) in newborns presents significant respiratory and feeding challenges.
  • Management strategies for BVCP are varied, with limited evidence on long-term efficacy.

Observation:

  • This retrospective study analyzed 8 newborns with BVCP, detailing their clinical course and treatment outcomes.
  • Multiple laryngeal procedures were performed, including laser cordectomy, arytenoidopexy, botulinum toxin injections, and laryngoplasty.

Findings:

  • No spontaneous recovery was observed in the study cohort.
  • Botulinum toxin alone was ineffective; however, arytenoidopexy combined with botulinum toxin yielded the best results.
  • Bilateral cordectomies were disappointing due to persistent vocal cord adduction.

Implications:

  • Early surgical intervention for BVCP is recommended due to the low likelihood of spontaneous resolution.
  • Combined arytenoidopexy and botulinum toxin injection appears to be a promising treatment approach.
  • Avoiding tracheotomy through timely surgical management is crucial for improved patient outcomes.
