Kidney Transplant I: Introduction01:28

Kidney Transplant I: Introduction

A kidney transplant is a surgical approach that involves replacing a non-functioning kidney with a healthy one from a donor. This procedure is often a treatment option for end-stage renal disease (ESRD) patients. The method requires careful recipient selection, including evaluating various medical and psychosocial factors. These criteria vary between transplant centers but generally include assessments of the patient's overall health, adherence to medical recommendations, and lifestyle...

Kidney Transplant II: Surgical Procedure

Preoperative ManagementThe primary goals of preoperative management in kidney transplantation are to optimize the patient’s metabolic state and prepare them for surgery through diet adjustments, necessary dialysis, and tailored medical treatment. This phase also involves comprehensive infection screening and patient education about the surgical procedure and postoperative care to improve outcomes and adherence.Medical ManagementA comprehensive evaluation is required for both the living donor...

Kidney Transplant III: Nursing Management

Postoperative Nursing Management for Kidney Transplant PatientsPostoperative nursing management care includes monitoring the surgical site, encouraging early movement, and promoting lung health through breathing exercises. Nurses also administer prescribed medications like H2-blockers, such as famotidine, or proton pump inhibitors, like omeprazole, to help prevent gastrointestinal ulcers and bleeding. Fungal infections in the mouth and bladder can result from immunosuppressive and antibiotic...

Pharmacokinetics in Pediatric Patients: Drug Excretion

In pediatric medicine, understanding the renal function and drug elimination nuances is crucial for administering safe and effective treatments. Newborns, in particular, display markedly slower renal functions than adults, profoundly affecting how drugs are cleared from their bodies. This slower drug clearance requires clinicians to extend the dosing intervals for many medications to prevent drug accumulation and toxicity while ensuring therapeutic efficacy. One key area where these adjustments...

Bone Marrow Sampling and Transplants

Bone marrow transplant is a potential cure for several diseases, including cancer and specific genetic disorders. Notably, this procedure is applicable for patients suffering from aplastic anemia, certain types of leukemia, severe combined immunodeficiency disease (SCID), Hodgkin's disease, non-Hodgkin's lymphoma, multiple myeloma, thalassemia, sickle-cell disease, and certain cancers.
The transplant begins with high doses of chemotherapy and radiation treatment, which aim to destroy the...

Summary

Pediatric renal transplantation offers excellent outcomes for end-stage renal disease (ESRD). Living donor transplants and older recipients showed higher graft survival rates, demonstrating the success of this life-saving procedure.

Area of Science:

  • Pediatric Nephrology
  • Transplantation Surgery
  • Immunology

Background:

  • End-stage renal disease (ESRD) in children necessitates renal transplantation as the primary treatment.
  • Pediatric ESRD patients often present with complex medical histories and high-risk factors.
  • This study evaluates the outcomes of renal transplantation in a pediatric cohort.

Purpose of the Study:

  • To analyze the results of renal transplantation in children with ESRD at a single center.
  • To identify factors influencing graft survival and patient outcomes.
  • To assess the long-term efficacy of renal transplantation in pediatric ESRD.

Main Methods:

  • Retrospective analysis of 86 pediatric renal transplant recipients (November 1993 - June 2006).
  • Data collected on donor type (living vs. cadaveric), recipient age, high-risk factors, and immunosuppression protocols.
  • Outcomes assessed included surgical complications, rejection episodes, infections, graft loss, and survival rates.

Main Results:

  • High 1- and 10-year actuarial recipient survival rates (99% and 98%).
  • 10-year graft survival rates were 98% for living donor transplants (LDTx) and 64% for cadaveric donors (CDTx).
  • Older recipients (>10 years) had better graft survival (86%) than younger children (<10 years) (75%).

Conclusions:

  • Renal transplantation is a highly effective treatment for pediatric ESRD.
  • Living donor kidney transplantation and transplantation in older children are associated with superior graft survival.
  • Pediatric renal transplant recipients with functioning grafts achieve full rehabilitation.
