Summary

Endoscopic transnasal sphenoidotomy (ETNS) is effective for inflammatory sphenoid sinus disease (ISSD). ETNS without ethmoidectomy suits isolated ISSD, while ETNS with ethmoidectomy is needed for concurrent paranasal sinus issues.

  • Disease staging based on CT findings using a standardized system.
  • Main Results:

    • Postnasal drainage was the most frequent symptom (88.1%).
    • Complications occurred in 19% of patients, including hemorrhage and injury to the lamina papyracea.
    • Isolated sphenoid disease (11.9%) and unilateral disease (38.1%) were noted, with disease stages ranging from 1 to 3.

    Conclusions:

    • ETNS without ethmoidectomy is favorable for isolated ISSD.
    • ETNS with ethmoidectomy is necessary for concurrent disease in other paranasal sinuses.
    • Surgical technique selection should be guided by ISSD extent and CT staging.
