A 5-day home trial of auto continuous positive airway pressure (CPAP) is optimal for initiating treatment in obstructive sleep apnea patients. Shorter trials are less cost-effective and have higher failure rates.

Conclusions:
- A 5-day autoCPAP trial is the optimal duration for initiating therapy in selected OSA patients.
- While a 1-day trial can determine CPAP pressure needs, it is not cost-effective and has a high failure rate.
- Optimizing autoCPAP initiation duration improves treatment outcomes and resource utilization.
