Summary

  • A 54-year-old male presented with a 4-month history of right arm and leg hypoesthesia, pain, and weakness.
  • MRI revealed an intradural extramedullary lesion at the foramen magnum and C1.
  • Histopathological and immunohistochemical analysis confirmed a melanotic schwannoma.

Findings:

  • Total resection of the melanotic schwannoma was successfully performed.
  • The patient experienced no local recurrence over a 2-year follow-up period.
  • Melanotic schwannoma exhibits ultrastructural Schwann cell features and melanin production.

Implications:

  • Complete surgical resection is the recommended management for melanotic schwannoma.
  • Long-term follow-up is essential to monitor for any potential recurrence.
  • Distinguishing melanotic schwannoma from malignant melanoma is critical for appropriate patient management.
