Purpose of the Study:

  • To analyze the incidence and bother associated with ejaculatory dysfunction (EJD) in men aged 50-78.
  • To identify factors that predispose men to developing incident EJD.

Main Methods:

  • A longitudinal study recruited 1661 men aged 50-78, excluding those with specific urological conditions.
  • Participants completed questionnaires assessing urological symptoms, sexual function (including EJD), and quality of life at baseline and three follow-ups over 6.5 years.
  • Objective variables of EJD included ability to ejaculate, ejaculatory volume, and painful ejaculation.

Main Results:

  • At baseline, 40.4% of men had EJD. The cumulative incidence reached 33.1% after 6.5 years.
  • Painful ejaculation bothered 40.6% of affected men, while reduced ejaculatory volume bothered 18.3%.
  • Multivariate analysis identified age, social impairment, and erectile dysfunction as significant predictors of EJD. Age and previous transurethral resection of the prostate (TURP) predicted reduced ejaculatory volume or anejaculation.

Conclusions:

  • Ejaculatory dysfunction (EJD) has a significant cumulative incidence and can be bothersome, particularly painful ejaculation.
  • Age, social impairment, and erectile dysfunction are key determinants of EJD.
  • Age and TURP are predisposing factors for reduced ejaculatory volume and anejaculation, with age being the most significant predictor.
