Area of Science:

  • Dermatopathology
  • Histiocytic Disorders
  • Pediatric Oncology

Background:

  • Indeterminate cell histiocytosis (ICH) is a rare and debated histiocytic disorder.
  • ICH shares features with both Langerhans cell histiocytosis (LCH) and non-LCH.

Observation:

  • A 12-year-old boy presented with a solitary verrucous papule on the glans penis.
  • The lesion exhibited characteristics of indeterminate cell histiocytosis.

Findings:

  • Neoplastic cells in ICH express S-100 and CD1a antigens.
  • Birbeck granules are absent in indeterminate cell histiocytosis.
  • ICH can manifest as cutaneous lesions in children and adults, with rare extracutaneous spread.

Implications:

  • This case underscores the importance of recognizing ICH in pediatric patients.
  • Reviewing morphologic and immunohistochemical features is crucial for accurate diagnosis.
  • Understanding ICH's relationship with LCH may refine classification and treatment strategies.
