Background:

  • Whole breast irradiation (WBI) is a standard treatment for early-stage breast cancer.
  • Conventional WBI often uses wedge-based techniques, which can lead to dose inhomogeneities.
  • Intensity-modulated radiotherapy (IMRT) offers potential for improved dose distribution and reduced toxicity.

Purpose of the Study:

  • To compare the acute and chronic toxicity of intensity-modulated radiotherapy (IMRT) with conventional wedge-based whole breast irradiation (WBI).
  • To evaluate the efficacy of IMRT in reducing radiation-induced side effects in breast cancer patients.

Main Methods:

  • A cohort of 172 patients with Stage 0-IIB breast cancer treated with lumpectomy followed by WBI was analyzed.
  • 93 patients received IMRT, while 79 patients received conventional wedge-based radiotherapy as a control group.
  • Median follow-up was 4.7 years, with toxicity assessed for acute and chronic effects.

Main Results:

  • IMRT significantly reduced acute Grade 2 or worse dermatitis, edema, and hyperpigmentation compared to wedges.
  • A trend towards reduced acute Grade 3 or greater dermatitis favored IMRT (6% vs. 1%).
  • Chronic Grade 2 or worse breast edema was significantly lower with IMRT. In patients with larger breasts, IMRT markedly reduced acute and chronic edema and hyperpigmentation.

Conclusions:

  • Intensity-modulated radiotherapy (IMRT) significantly decreases acute dermatitis, edema, and hyperpigmentation in whole breast irradiation.
  • IMRT leads to a significant reduction in the development of chronic breast edema compared to conventional wedge-based radiotherapy.
  • IMRT is a superior technique for whole breast irradiation, offering improved toxicity profiles.
