Summary

The Voluntary Step Execution Test, when performed with cognitive load, can identify elderly individuals at risk of falls. This dual-task test is a simple, safe method for fall risk assessment in older adults.

  • Balance and gait were assessed using the Berg Balance Test (BBS) and Timed Get Up and Go (TUG). VSET was administered under single and dual-task conditions.
  • Main Results:

    • No significant differences in BBS and TUG scores were found between fallers and non-fallers.
    • Single-task VSET showed no significant differences between fallers and non-fallers.
    • Dual-task VSET revealed significant increases in preparatory phase duration, swing time, and foot-contact time in fallers. Participants with dual-task execution times >1,100 ms had a fivefold increased risk of falling.

    Conclusions:

    • The Voluntary Step Execution Test under dual-task conditions is a sensitive measure for identifying elderly individuals at high risk of falls.
    • This simple and safe test provides valuable insights into fall risk beyond traditional assessments.
    • Dual-tasking challenges cognitive and motor functions, revealing subtle deficits indicative of fall risk.
