Rocky Mountain Spotted Fever01:26

Rocky Mountain Spotted Fever

Rocky Mountain Spotted Fever (RMSF) is a severe tick-borne illness caused by Rickettsia rickettsii, a Gram-negative, coccobacillary bacterium. This pathogen is an obligate intracellular parasite, requiring a host cell for replication. Transmission occurs through the bite of an infected tick. In the United States, the most important vectors are Dermacentor variabilis (American dog tick) and Dermacentor andersoni (Rocky Mountain wood tick), though other tick species may also serve as vectors.
Bacterial Phylum Spirochaetes01:30

Bacterial Phylum Spirochaetes

Spirochetes, unique bacteria in the phylum Spirochaetes, are gram-negative, motile, tightly coiled, slender, and flexible. They inhabit aquatic sediments and animals, with some causing diseases like syphilis. Spirochetes are classified into eight genera based on habitat, pathogenicity, phylogeny, and characteristics.Their distinctive motility arises from endoflagella, located within the cell’s periplasm. These endoflagella anchor at the cell poles and extend along the cell length, encased by a...

    Summary

Diagnosing Bartonella infection, like cat-scratch disease (CSD), is possible retrospectively using PCR on lymph node biopsies. Suspect CSD with persistent lymphadenopathy and feline contact history.

    Area of Science:

    • Infectious Diseases
    • Hematology
    • Microbiology

    Context:

    • Bartonella henselae is a common cause of cat-scratch disease (CSD).
    • Diagnosis can be challenging due to non-specific symptoms.
    • Hematological evaluation is often sought for persistent lymphadenopathy.

    Purpose:

    • To detail clinical and histological findings of Bartonella infection.
    • To evaluate diagnostic utility of serological and molecular methods.
    • To establish retrospective diagnostic criteria for CSD.

    Summary:

    • Retrospective analysis of 747 patients with lymphadenopathy identified 10 suspected Bartonella cases.
    • Polymerase chain reaction (PCR) on archival lymph node biopsies enabled retrospective CSD diagnosis.

  • Dynamic serological studies and PCR of biopsy specimens are recommended for Bartonella diagnosis.
  • Impact:

    • Highlights PCR's effectiveness for retrospective CSD diagnosis from archival samples.
    • Emphasizes clinical suspicion based on lymphadenopathy and feline contact.
    • Guides improved diagnostic strategies for Bartonella infections in hematology patients.
