Area of Science:

  • Ophthalmology
  • Corneal Surgery
  • Transplantation

Background:

  • Descemet stripping endothelial keratoplasty (DSEK) is increasingly favored for corneal endothelial dysfunction.
  • Understanding recent advancements is key to minimizing the learning curve and complications.

Purpose of the Study:

  • To review recent advances in DSEK techniques and instrumentation.
  • To highlight improvements in patient outcomes and safety profiles.

Main Methods:

  • Review of current literature on DSEK advancements.
  • Analysis of new techniques for graft preparation and insertion.
  • Evaluation of outcomes and complication rates.

Main Results:

  • DSEK yields excellent visual results with minimal refractive or topographic changes.
  • Successful treatment for various conditions including Fuchs' dystrophy and failed grafts.
  • Automated donor dissection and improved graft insertion techniques are noted.
  • Lower graft rejection rates compared to penetrating keratoplasty.
  • Graft detachment is the most common early complication, with improved management strategies.
  • Early trials of endothelial cell transplantation show promise.

Conclusions:

  • DSEK offers superior visual rehabilitation and safety compared to penetrating keratoplasty.
  • Ongoing technical evolution in DSEK continues to reduce complications and enhance outcomes.
