Background:

  • Exercise-based cardiac rehabilitation aims to enhance patient-reported outcomes (PROs), including quality of life (QoL) and symptoms of anxiety and depression.
  • Direct comparative data on the effectiveness of inpatient versus outpatient cardiac rehabilitation programs for PROs are lacking.

Purpose of the Study:

  • To compare the effectiveness of inpatient and outpatient exercise-based cardiac rehabilitation programs on patient-reported outcomes.
  • To assess the changes in health-related quality of life, anxiety, and depressive symptoms over a 3-month period.

Main Methods:

  • A non-randomized study involving 216 patients undergoing either inpatient (4-week) or outpatient (3-month) exercise-based cardiac rehabilitation.
  • Patient-reported outcomes were measured using the MacNew Heart Disease Health-related Quality of Life questionnaire and the Hospital Anxiety and Depression Scale at baseline, 1 month, and 3 months.
  • A usual care group (n=67) was included to monitor natural changes in PROs.

Main Results:

  • Both inpatient and outpatient cardiac rehabilitation showed significant improvements in QoL and reduced anxiety/depressive symptoms by 1 month compared to baseline.
  • While QoL initially improved more in the inpatient group, it declined by 3 months, whereas it continued to improve in the outpatient group.
  • Anxiety and depressive symptoms were maintained at 3 months only in the outpatient group, unlike the inpatient group. The usual care group showed no significant changes.

Conclusions:

  • Both inpatient and outpatient exercise-based cardiac rehabilitation yield significant short-term (1-month) improvements in PROs compared to usual care.
  • Outpatient cardiac rehabilitation demonstrates superior long-term effectiveness, leading to sustained improvements in QoL and mental health outcomes.
  • Combining inpatient with subsequent outpatient cardiac rehabilitation is recommended to optimize and stabilize patient-reported outcomes.
