Purpose of the Study:

  • To elucidate the interrelation of pain and depression by examining brain activity during thermal pain stimulation.
  • To identify differences in cerebral responses to varying thermal pain intensities between MDD patients and healthy individuals.

Main Methods:

  • Assessed individual thermal pain thresholds.
  • Utilized an fMRI-compatible thermode to deliver thermal stimuli (37°C, 42°C, 45°C) to the right arm.
  • Processed and analyzed fMRI data using Statistical Parametric Mapping 2 (SPM2).

Main Results:

  • MDD patients exhibited significantly increased thermal pain thresholds compared to controls.
  • Both groups showed comparable blood oxygenation level-dependent (BOLD) signal increases in pain matrix structures.
  • MDD patients displayed hyperactivation in the left ventrolateral thalamus and right prefrontal cortex (VLPFC, DLPFC) at 45°C, with stronger parametric BOLD signal increases in these areas and the insula.
  • Symptom severity in MDD patients positively correlated with BOLD signal in the left ventrolateral thalamus.

Conclusions:

  • Cortical pain matrix structures are similarly activated in depressed patients and healthy subjects.
  • Increased prefrontal and lateral thalamic activation in MDD patients during painful stimuli presentation.
  • These findings may explain the reduced thermal pain perception observed in individuals with depression.
