Summary

Occult neck metastases are common in supraglottic carcinoma. Selective neck dissection and radiotherapy are effective in managing these hidden metastases, preventing further spread.

Main Results:

  • Occult cervical node metastases were identified in 18% of patients.
  • Ipsilateral metastases were most frequent (77%), but bilateral and contralateral spread also occurred.
  • Postoperative radiotherapy in patients with confirmed metastases resulted in only 1% subsequent metastasis development, demonstrating high efficacy.

Conclusions:

  • Supraglottic carcinoma frequently exhibits occult cervical metastases.
  • Selective neck dissection of levels II-IV is recommended.
  • Locoregional radiotherapy should follow for histologically confirmed nodal micrometastases to ensure effective disease control.
