Background:

Renal infarction, particularly of the lower pole, is a known complication post-renal transplantation.
Thrombosis of renal arteries can lead to ischemic injury and subsequent infarction.

Observation:

An unusual case of lower pole renal infarction and lower calyx perforation is presented.
The infarction was attributed to thrombosis of a specific lower polar artery.

Findings:

The patient's condition was successfully managed with partial nephrectomy.
This nephron-sparing surgical approach preserved renal function.

Implications:

Partial nephrectomy is an effective treatment for selected cases of renal infarction with perforation.
Nephron-sparing surgery is crucial for maintaining graft function in renal transplant recipients.
