Background:

  • Primary bilateral adrenal lymphoma is an extremely rare condition.
  • Non-Hodgkin lymphoma (NHL) can rarely manifest as bilateral adrenal masses.

Observation:

  • Three cases of primary bilateral adrenal lymphoma (diffuse medium-sized cell type NHL) are presented.
  • Patients presented with anemia or general malaise, with bilateral adrenal masses detected via CT.
  • Two patients experienced transient responses to chemotherapy, followed by disease progression and death.

Findings:

  • Pathological diagnosis confirmed diffuse medium-sized cell type non-Hodgkin lymphoma in all three cases.
  • Chemotherapy showed initial efficacy in reducing adrenal masses, but relapse and progression were observed in two patients.
  • One patient achieved complete remission with combination chemotherapy.
